The Family Assessment Planning Team
The Family Assessment and Planning Team (FAPT) develops service plans for eligible youth and families and reviews cases funded by the Comprehensive Services Act to determine progress, prepare transition plans, and adjust services as needed.
Team members are appointed by the CSA Committee. The Family Assessment and Planning Team includes a representative from the following agencies:
- Charlottesville Department of Social Services
- Albemarle County Department of Social Services
- Charlottesville Public Schools
- Albemarle Public Schools
- Region Ten
- Juvenile Court Services Unit
The Team also includes a parent representative.
The Family Assessment and Planning Team studies and discusses information from case managers, parents and/or guardians, psychological reports, school records, and other pertinent data sources. The child's family is encouraged to attend and participate in the FAPT meeting.

- FAPT Checklist- list of items required to schedule and present a FAPT case.
- FAPT Consent for the Release of Confidential Information Form - this form, signed by a parent or guardian, must be received by the CSA Coordinator before a case can be heard by the FAPT.
- FAPT Consent for the Release of Confidential Information for Alcohol and Drug Service Form- (required only if applicable) the youth AND the parent or guardian must sign this form.
